Complete Information for medical
and palliative care intervention.
Eleven Screens of Information.
Dossier is conveniently organized by topic, as
follows.
1. General Information displays
all the client’s basic data, to which are added
the names of the different professionals intervening
in the file, the reference hospital and all pertinent
information.
2. General Care keeps the diagnosis,
notes, treatments and vital signs. This is also
where the necessary tabs for this client are chosen.
Each Cardex client file is thus adapted.
3. Medication Follow-Up screen
lists all drugs to be administered or taken by
patient with posology, dates and secondary effects,
along with Pharmacist information.
4. Palliative Care is a screen
where the effects on the patient’s daily life
can be documented. A click on a body part of a
figure marks the sore area. Pain history is also
kept here.
5. Wound Care. All information
on wounds is put in this screen. It allows also
the determination of check-up frequency and appointments
planning. The same goes for dressings.
6. Intravenous/Subcutaneous screen
keeps frequency and all information pertaining
to this type of care: treatment, catheter, plug,
dressing, tubing, irrigation and method of administration.
7. Health Care Plan tab is used
to put the objective, intervention, evaluation
date and results date, for each diagnosis established
on a certain date. The number is unlimited.
8. Laboratory. The laboratory
analyses are planned in this tab. In the «Tests»
button are selected the analyses to be made. Their
frequencies and dates are then entered. The Care
Plan function is available.
9. Psychosocial Situation screen
is a drawing board in which a genogram can be
drawn or inserted. Space to put information on
the client’s natural aides is also provided here.
10. Teaching tab helps follow
the instructions given to patient.
11. Equipment Loan window holds
all loaned items along with the loaned and returned
dates.
